Where the adapting tempo and difficulty left off

Both systems keep a per-song ladder from 1 to 11: where the last run ended, the highest rung reached, and where the next run will start.

Dynamic tempo and dynamic difficulty each keep a ladder per song and per part: the rung I finished on, the best rung I have reached and where the game will start me next time. It writes the first row when I finish a song with either of them switched on.
